Patient journal
You have the right to see which documents your patient record consists of and who has accessed them. The time limit for a reply is four weeks from the receipt of the inquiry.

Below you will find the forms you need to print, fill out, and sign. You can return them to us in the following ways:
- Via eDialog: Upload and send the signed form as an attachment.
- By mail: Send the signed form to Helse Nord-Trøndelag HF, Postboks 333, 7601 Levanger.
Please note that, for security reasons, we cannot provide copies of patient records and logs via email.
Access to Patient Records
All patient treatment must be documented in a patient record. You have the right to receive a copy of your medical record.
Printing of Journal Log
You can request a printout of the log showing who has accessed your electronic patient record.
Limitation on Access to Information in Patient Records
If necessary to ensure appropriate health care, health care professionals may share your health information. You have the right to object to this. In this case, you must inform the treatment center.
Correction/Deletion of Journal
Information or statements in the record can be deleted if they are incorrect, misleading, feel burdensome, or are obviously not necessary to provide healthcare.
